Get A Free Waiver Form

A waiver permits you or a third party to intentionally relinquish a right or claim to compensation or relief. Generic templates are available through online legal document suppliers, and can be downloaded for use. Before using a waiver form, read the instrument from start to finish. Ensure that it contains the language you want, and covers the activities you want covered.


Instructions


1. Visit your state's court website. If you seek to bring legal proceedings against a defendant, request the appropriate waiver form from the county clerk. Generally, waiver forms are available for download online. Charges may apply if you obtain the forms at the courthouse.


2. Visit an online legal document supplier. Not all online legal document supplies offer free downloads, so check beforehand. Generally, online legal document suppliers charge a fee if an in-house consultant prepares or offers expert advice regarding the completion of a form.


3. Choose from various blank waiver templates, including application fee waiver forms, medical insurance fee waiver forms, medical release waiver of liability, waiver of liability forms, pet waiver of liability forms, and legal liability waiver forms.


4. Analyze the language contained within the waiver. Look for specific language about the scope of liability and level of risk -- for example, "I understand and fully accept that there are risks involved. I hereby agree to accept any and all risks of injury or death." If the waiver does not contain the language you want, select a more effective template.


5. Download the free waiver form. Check that the form is complete after printing the job. Each page must be present, especially the signature and signing date page.


6. Fill in the form completely and initial where appropriate. Have all parties sign the form as well as print a hand signature and signing date.


7. Have the waiver notarized by a notary public to strengthen the enforceability of the form.
